Sample Submission

Samples can be submitted daily to DonLevy Laboratories. Our courier department offers sample pick up in the greater Chicago area. Please contact us to determine if we can provide sample pick up from your location.

Outside of the Chicago area, samples can be shipped to our laboratory in Crown Point, Indiana, by a courier of choice. Please contact us for assistance in determining the most economical and efficient means of transporting your samples. DonLevy Laboratories can also return coolers upon request.

Our sample administration group is responsible for input of data into the Laboratory Information Management System (LIMS). A sample analysis request form (SARF) is required to accompany samples sent to our laboratory. This form allows our clients to specifically request the tests to be performed.

The sample receiving process involves the development of a work or project order based on the information provided in the SARF. A certificate of analysis (CoA) is generated from the completed work or project orders.

DonLevy Laboratories provides CoAs electronically in a PDF file. All documentation, including SARFs, is kept in storage at DonLevy Laboratories for three years unless an additional amount of time is specified.
